This form sets out exactly what we are asking for, how we will use your logo, for how long, and how you can withdraw your consent at any time. Filling this form is entirely voluntary. Please review the information below and submit.

3. What we are asking for
We are asking for two related things:
- A licence to use your company name, logo and (optionally) a short testimonial in the marketing channels listed in section 4.
- Your consent, under Article 6(1)(a) GDPR, to process the personal data on this form (the contact person's name, role, email address and signature) for the purpose of administering and evidencing this consent.
A company logo on its own is generally not personal data under the GDPR. However, where the logo or company name relates to an identifiable individual (for example a sole trader), GDPR also applies to the logo itself. By submitting this form you confirm that the person granting permission has the authority to do so on behalf of the organisation named above.
4. Permitted uses
Please tick the channels and materials in which you allow TolaData to display your company name and logo. You can tick as many or as few as you wish.
5. Logo files and brand guidelines
To make sure we represent your brand correctly, please share the logo files and any brand guidelines you would like us to follow. If you do not have time to attach these now, we will follow up by email.
6. Time frame of use
Please choose how long you would like this consent to remain valid. You can withdraw or amend it at any time (see section 8).
After the agreed period or after we receive a withdrawal of consent, TolaData will:
- Stop using your logo in any new materials produced from that date onwards.
- Remove your logo from our website and from materials we control (e.g. live decks, online ads) within a reasonable period, normally no longer than 30 days.
- Note that copies of materials already distributed offline (printed brochures, archived recordings, third-party caches) cannot always be recalled, but no further distribution will take place.
7. Geographic scope and international transfers
Some of our marketing channels (e.g. social media platforms, cloud-based design tools) are operated by providers that may store or process data outside the European Economic Area, including in countries such as the United States. Where this is the case, we rely on appropriate safeguards as required by Articles 44–49 GDPR (such as Standard Contractual Clauses or an applicable adequacy decision).
8. Your right to withdraw consent
You can withdraw your consent at any time, with effect for the future, without giving any reason. Withdrawing your consent does not affect the lawfulness of any processing or use that took place before the withdrawal.

This section sets out how TolaData handles the personal data contained in this form.
Personal data we process
- Identifying and contact data of the signatory: name, role, organisation, email address, and date.
- Information about your consent choices on this form (channels, time frame, geographic scope).
- Technical metadata captured at the time of submission (IP address, user agent, timestamp) for accountability purposes under Article 7(1) GDPR.
Purposes and legal basis
- Documenting and administering your consent for logo and brand use – legal basis: your consent, Article 6(1)(a) GDPR.
- Demonstrating that valid consent was obtained (accountability) – legal basis: our legitimate interest in being able to evidence compliance, Article 6(1)(f) GDPR, and our legal obligation under Article 7(1) GDPR.
Recipients
- TolaData employees who need access for marketing, legal or compliance reasons.
- IT service providers acting as processors under Article 28 GDPR (e.g. hosting, email, CRM, document storage).
- Marketing platform providers strictly as needed to display the logo on the channels you have approved.
We do not sell your personal data and we do not use it for any purpose other than those listed above.
Storage period
- We keep this signed form for as long as we use your logo, plus a further three (3) years after the end of use, in line with general statutory limitation periods, so we can demonstrate that consent was given.
- If you withdraw your consent earlier, we will continue to store the form, marked as withdrawn, for the same retention period in order to evidence that we acted on your withdrawal.
Your rights
Subject to the conditions of the GDPR, you have the right to:
- Access the personal data we hold about you (Article 15).
- Rectify inaccurate personal data (Article 16).
- Erasure of your personal data (Article 17).
- Restrict processing (Article 18).
- Data portability (Article 20).
- Object to processing based on legitimate interests (Article 21).
- Withdraw consent at any time (Article 7(3)), as described in section 8.
- Lodge a complaint with a supervisory authority (Article 77), in particular in the EU/EEA Member State of your residence, place of work or the place of the alleged infringement.

Automated decision-making
We do not use the data on this form for any automated decision-making or profiling within the meaning of Article 22 GDPR.
10. Your consent and signature
By ticking the boxes and submitting below, I confirm that:
- I am authorised to grant the rights described in this form on behalf of the organisation named in section 1.
- I have read and understood the information in sections 1–9, including the data protection information in section 9.
- I grant TolaData a non-exclusive, royalty-free, revocable licence to use our company name and logo in the channels and within the time frame and geographic scope I have ticked above.
- I have given my consent freely, informed, specifically and unambiguously, and I understand that I can withdraw it at any time with effect for the future.
